1. The service

EasyCheckUp is a digital preventive health platform. It lets you complete an adaptive health questionnaire, receive informational preventive suggestions and purchase a PDF report, generated and downloaded on your own device — with the results of that assessment saved to your account —, designed to support a conversation with your doctor.

EasyCheckUp is strictly an informational, preventive tool. It does not provide diagnoses, does not prescribe treatments and is neither a medical device nor a telemedicine service.

2. No substitute for medical assessment

The suggestions provided never replace assessment, diagnosis or follow-up by qualified health professionals.

In case of intense symptoms, sudden worsening or an emergency, contact SNS 24 (808 24 24 24) or 112 immediately. Do not use the platform for urgent situations.

3. Eligibility and account

The platform is intended for people aged 18 or over. Creating an account requires a valid email address and a password. You are responsible for keeping your credentials confidential.

4. Truthfulness of answers

Suggestions and the report are generated exclusively from the answers you provide. The truthfulness, accuracy and completeness of those answers are entirely your responsibility.

The platform accepts no responsibility for suggestions or reports affected by incorrect, incomplete or false answers.

5. Answers are not stored

By design, questionnaire answers are never stored by the platform. They exist only in memory during your session. If you close or refresh the page, the answers are lost and you will need to repeat the questionnaire.

When you buy the report, the generated results — the suggestions, the exam plan and the risk level, never your answers — are saved to your account, where you can review them, download the PDF again or delete them. Nothing is sent by email. Without a purchase, nothing is stored.

The downloaded PDF is your record of the report. We recommend keeping it somewhere safe.

6. Payments, invoicing and refunds

The PDF report costs a one-time €2.00 (VAT included), with no subscription. Payment is processed by Stripe; the invoice is issued through InvoiceXpress with the tax number you provide at checkout (optional; without it, the invoice is issued to a final consumer).

If, after a confirmed payment, you do not receive the report for a reason attributable to the platform, you are entitled, at your choice, to repeat the process at no cost or to a full refund. Contact us at the support email.

By paying, you consent to the immediate supply of the digital content (the report) and acknowledge that, upon delivery, you lose the withdrawal right provided for digital content under Portuguese Decree-Law 24/2014. This consent does not affect the rights described in the previous paragraph.

7. Intellectual property

The platform, the questionnaire, the suggestion rules and the report format are the property of EasyCheckUp. The generated PDF is intended for your personal use and for sharing with the health professionals of your choice.

8. Changes

These terms may be updated. The version in force is the one published on this page, with the update date shown. Relevant changes will be communicated reasonably.

9. Governing law, dispute resolution and contact

These terms are governed by Portuguese law. For any question, contact info@easycheckup.io.

For consumer disputes you may turn to an alternative dispute resolution (ADR) body. For consumers resident in Portugal, the competent body is the Centro Nacional de Informação e Arbitragem de Conflitos de Consumo (CNIACC, www.cniacc.pt) and the territorially competent consumer arbitration centres.
